Northreach Society, the service provider for the Coordinated Care Campus, has notified the City of Grande Prairie that they intend to revise their service delivery plan away from supportive housing and towards a primary focus on healthcare supports for the city’s most vulnerable.  This change will occur at the end of June and includes carefully planned provisions with continued positive collaboration between Northreach and the City during and after the transition to a new Coordinated Care Campus service provider.

“Northreach Society has been a qualified and committed partner in the development of the Coordinated Care Campus, and we have been pleased to work alongside them to provide innovative solutions to lessen the impacts of homelessness and ultimately save lives,” said Grande Prairie Mayor, Jackie Clayton.  “We thank Northreach Society for their diligence to service and care for some of our community’s most vulnerable.”

“The City is a valuable partner, and we commend their efforts to involve expert organizations in their solutions for housing and homelessness in the community,” said Northreach Society Executive Director, Dalyce Sather-McNabb. “Although the scope of Northreach Society is set to change, our new direction will continue to see us working alongside the City in administering support and providing health services to the clients of the Coordinated Care Campus.”

To facilitate a seamless transition to a new service provider, the City and Northreach Society will implement measures that ensure all clients of the Coordinated Care Campus continue to receive quality service.  The City does not anticipate any interruptions as a result of this change.  Likewise, the Request for Proposal (RFP) process to find a new service provider contains measures enabling the successful proponent to work with the existing Coordinated Care Campus staff, ensuring continuity of care for all clients and avoiding adverse impacts on staff.
