This form is to be used when filing an assessment complaint.

Fees and Timelines

This form must be submitted by the date noted on your Notice of Assessment. Fees for filing a complaint are set out in Bylaw C-1395 - Fees, Rates and Charges Bylaw.

Application Requirements

The form must be filled out completely or it will be deemed invalid.
